The custodian is responsible to manage the case activity and engage necessary treatment team members to achieve permanency as timely as possible and as stated in Responsibilities of the Custodian in 624-05-12. In addition, the custodian’s responsibilities may also include but are not limited to:

 

Case Manager:

  1. Complete the referral paperwork in its entirety inclusive of the Comprehensive Case Coordination Referral (SFN 674), Universal Application (SFN 824), Foster Care Child Needs Assessment and Agreement (SFN 1865), most recent PCPA, Genogram, and any additional supporting documentation.

    1. Supporting documentation may include a recent psychiatric evaluation, therapy notes, diagnostic evaluation, treatment plan, specialist evaluation, IQ Testing, IEP, police reports, etc.

  2. Submit referral documentation to:

    1. CFS Case Management Field Service Specialist Unit

  3. Engage in Comprehensive Case Planning in conjunction with the Comprehensive Case Coordinator through a "Pitch-In" model.

  4. Manage overall case activities and day to day needs of the child as the primary point of contact for the child, parents, providers, and treatment team members.

 

Zone Supervisors Role:

  1. The Human Service Zone Supervisor will support agency staff in delivering their responsibilities related to CCC recommendations. They will provide coverage, support, and technical assistance to agency staff to ensure successful discharge and permanency planning for the child receiving CCC Programming.

  2. Engage in planning with the Case Coordinator, primary case manager, and other identified treatment team members to remain informed with case progress.

 

Zone Directors Role:

  1. The Human Service Zone Director or designee, such as a Family Service Manager, will partner and engage in at least monthly planning for the child to stay involved in the high-fidelity case planning for the child.

 

Comprehensive Case Coordinators Role:

  1. Review all referrals to the Comprehensive Case Coordination program.

  2. Provide direct supervision and support to agency case manager only as it pertains to the child accepted in the CCC program.

  3. Facilitate meetings with the child’s care team to monitor ongoing progress and follow through of treatment team members.

  4. Complete a comprehensive overview of the case as it relates to placement history, family engagement, relative searches, and referrals to services.

  5. Complete a Crisis Response Plan within the first week for each child in their current placement.

 

If at any time either party (Zone or Comprehensive Case Coordinator) wishes to exit the program, they have the right to do so. A closing meeting will be held to provide current overview of services and status of the case. The Zone Director, Zone Supervisor, Zone Case Worker, and assigned CM FSS are required to participate.
